Use the following forms for porting between Wireless providers. Fax forms can be used as a backup when an electronic connection cannot be made or is not availalble or in the event of a provider’s system outage.

Wireless Porting Fax Forms
Single Line Wireless Port Request (WPR) Use these forms to send a port-in request to a trading partner. Single Line WPR or SUP – page 1 (#27431) Single Line WPR or SUP – page 2 (#38841) Single Line Wireless Port Request Response (WPRR) Use this form to send a response to a trading partner for a port-out request. Single Line WPRR (#28891) Mulitple Line Wireless Port Request (WPR) Use this form to send a port-in request for more than one line to a trading partner. Multi-line WPR (#28901) Mulitple Line Wireless Port Request Response (WPRR) Use this form to send a response to a trading partner for a port-out request of more than one line.
